Permission Form

KIDS ASK AUTHORS PODCAST PERMISSION SLIP

 The Kids Ask Authors team (headed by author/illustrator Grace Lin) will be using podcasts to share students’ questions, book reviews and stories.  We will be collecting these recording and sharing them on the Kids Ask Authors podcast. 

For safety reasons we will not use last names in audio recordings that are uploaded to the Internet. Unless requested, we will either use fictional first names or code names. 

We need a legal guardian’s permission to include a child in this project.   Please complete this permission slip and include it with your child’s recording (you can send a photo of it).  We will not include your child in this project unless we have your permission.
